Login or Create an Account: www.myprovidence.com 

If you do not have an account, select "Create account" to register.

To create an account, Employees will need date of birth and zip code, and member ID or first name and last name. If you do not have your Member ID, when asked "How are we identifying you today?" click on Member ID/Group ID to switch the option to First Name / Last Name.

Once logged in, the home page should present the ability to "View Digital Card" or you can navigate to "My Tools" and view your member ID card.
