How to Obtain Moda Health Insurance Card

Log in or Create an Account: https://www.modahealth.com/dashboard/login

If you do not have an account, select "Create an Account" and follow the steps.

To create an account employees will need Subscriber ID, First name, Last name and Date of Birth. If you do not have your subscriber ID, call the Member Dashboard 888-374-8907 or email [e-mail].

Once logged in, you can access your digital card from the 'Member Shortcuts' box on the Member Dashboard homepage, or the 'Resources' tab.

Smartphone users, you can download your ID card from the Moda Health Mobile ID card app. Search for 'Moda Health eCard App' in your app store. Once downloaded enter your mobile pin code and Member ID to launch the app.
